How to Apply
- Applications are invited from in-house contractual employees of NHM working in the same post under OSH&FW Society in other districts. Fresh candidates are not eligible to apply.
- Candidates must apply in the prescribed application form, which is available on the official website www.gajapati.odisha.gov.in.
- The application form must be submitted along with a No Objection Certificate (NOC), a continuation certificate for uninterrupted service in the same post, a color passport size photograph, and self-attested photocopies of all relevant certificates and mark sheets.
- Applications should be sent by Registered/Speed Post to: Chief District Medical & Public Health Officer, At/Po-Paralakhemundi, Gajapati, 761200.
- The application must reach the specified address on or before 31.10.2025 by 05:00 PM.
