All India Institute of Medical Sciences, Rajkot Recruitment 2025 – 06 NMHS Field Data Collector Posts

Interested and eligible candidates must submit their application as follows:

1. Complete the prescribed application format (available in the advertisement).
2. Prepare one set of self-attested photocopies of the following documents:
   • Date of Birth proof
   • Photo ID Proof (PAN Card, Aadhaar, Voter ID, Driving License, etc.) with Date of Birth.
   • Educational qualification- 10th/12th/graduate/ PG degree/ diploma certificate etc as applicable
   • Mark sheets of respective educational qualifications.
   • PhD certificate (if applicable)
   • Registration certificate (if applicable).
   • Experience certificates (if applicable).
3. Submit a soft copy of the application along with scanned copies of all attachments using the Google form link: https://forms.gle/p9zK5VtY749RRCCB9 by 2025-12-03 till 5 PM.
4. A physical copy of the form and all documents, with xerox copies and with original, are required during document verification.
5. Candidates must report at 9 AM for the written test/walk-in interview at Research Cell, 3rd floor Academic block, AIIMS Rajkot on 2025-12-04. Candidates reporting after 9 AM will not be considered.
