Chief Medical Officer, Kushinagar Recruitment 2026: Apply for 01 Contractual Dental Surgeon (BDS) Vacancy

A fixed honorarium of Rs. 50,000/- per month will be provided. For doctors who worked during COVID-19 and possess QR code-based experience certificates from the district, their appointment will be at the honorarium payable during their COVID-19 deployment period.

Interested and eligible candidates must submit their application forms by registered post to the Office of Chief Medical Officer, Kushinagar. The application should reach the office by 05:00 PM on March 25, 2026 (during working days and hours). Applications received after the deadline will not be considered.

• The application form must clearly state the 'Name of the Applied Post' and 'Program Name' at the top.
• Include personal details such as Name, Father's Name, Address, Reservation Category, Mobile Number, and Email.
• Provide all academic details (examination, passing year, Board/University, subjects, total marks, marks obtained, percentage).
• Enclose self-attested copies of all educational and technical mark sheets, certificates, experience certificates, caste certificate, domicile certificate, and registration certificate.
• Affix a recent passport-size photograph on the top right corner of the application form and sign it.
• Failure to attach self-attested copies of required qualification and experience certificates will lead to the rejection of the application.
• The envelope must be superscribed with the 'Name of the Applied Post' and 'Program Name' in block letters.
